Notice*: It is recommended to do core rotations first before doing elective rotations, especially for foreign medical graduates.

Upon Your acceptance Into A Clinical Rotations Through Alpha Med Omega, The Following Documents Are Required, And must be Submitted to AMO prior To Starting A Rotation.

  • OSHA.
  • HIPAA.
  • Varicella.
  • Curriculum vitae.
  • Malpractice insurance.
  • Letter of good standing.
  • Influenza Immunization.
  • 10 Panel Drug screening.
  • Hepatitis B (unless positive).
  • Photocopy of an identification card.
  • Rubella (proof of vaccination or titer).
  • Mumps (proof of vaccination or titer).
  • Measles (proof of vaccination or titer).
  • COVID-19 (proof of vaccination or titer).
  • Copy of student’s School Photo ID Badge.
  • Basic Life Support for Healthcare Providers.
  • Confidentiality Statement Signed and attached.
  • General Physical Exam within the past one year.
  • Criminal Background check must be within a year.
  • PPD (within the past six months, if positive then need an X-ray results).

Students must wear their Medical school ID badge at all times during rotations.
